What documents do I need to apply?

Applicants may be required to provide:

  • Social Security Card or ITIN documentation for all household members
  • Birth certificates for minors
  • Proof of income
  • Bank statements and asset information
  • Additional application and compliance forms

What proof of income is accepted?

Accepted income documents may include:

  • Current pay stubs
  • Social Security or disability award letters
  • Pension or retirement income
  • Other verifiable sources of income

How many pay stubs are required?

  • Weekly pay: 9 current consecutive pay stubs
  • Bi-weekly pay: 6 current consecutive pay stubs

What asset information is required?

Applicants may need to provide:

  • 6 months of checking account statements
  • Current savings account statements
  • Documentation for retirement accounts, investments, or additional assets

Do all adults need to complete forms?

Yes. All household members 18 and older may be required to complete and sign applicable application, authorization, and certification forms.

Are students required to provide additional information?

Full-time students may be required to provide additional documentation, such as school schedules or student certifications.

How long does the application process take?

Processing times may vary depending on how quickly all required documents are submitted and verified.

Can incomplete applications be processed?

No. Missing documents or unsigned forms may delay application processing.
